What we collect
If you use the contact form on this website, we collect the name, email address, optional phone number, and message you choose to provide. That is the only information this website asks you for.
Our web host records standard technical information such as IP address and browser type as part of delivering and protecting the site. We do not use Google Analytics, advertising pixels, or third-party tracking scripts on this website.
How we use it
We use what you send through the contact form solely to respond to your question and, where appropriate, to follow up about care at our practice. We do not sell, rent, or trade your information, and we do not use it for advertising.
Where it goes
Contact-form messages are delivered directly to the practice's own Microsoft 365 email. They are not routed through a third-party marketing or CRM service. Our website is hosted on Cloudflare, which also provides the spam protection on the form.
Please do not send medical details through this form
Email and website forms are not a secure or encrypted channel. Please do not include your date of birth, diagnoses, medications, test results, or other sensitive medical information in a website message. For anything clinical, call the office at (727) 807-6900 or use the Patient Portal, which is secure.
If you do send medical information through the form anyway, we treat it with the same confidentiality as any other patient information we hold, under our Notice of Privacy Practices.
How long we keep it
Contact-form messages are retained in the practice's email only as long as needed to respond and to keep a record of the correspondence. Where a message becomes part of your medical record, it is retained under the same rules as the rest of your record.
Children
This website is not directed at children and we do not knowingly collect information from children through it. Care for minor patients is arranged by a parent or guardian through the office.
Links to other sites
This site links to outside services such as our Patient Portal, Healthgrades, and our social media pages. Once you follow those links, their privacy policies apply, not ours.
Changes to this policy
If we change this policy we will update the date at the top of this page.
