Semaglutide and tirzepatide are two different molecules, not a basic and a premium version of the same drug. Semaglutide acts on one receptor target; tirzepatide is a newer dual-agonist that acts on two — and in the head-to-head trial that compared them directly, tirzepatide produced greater average weight loss over 72 weeks. It also costs more to source. That is the difference you see in the monthly price, not a markup.
